COSHH - Respiratory Surveillance Medical Form - Paper Version

The Control of Substances Hazardous to Health (COSHH) Regulations 1994. Require employers to carry out health surveillance for employees exposed to chemicals, animals or other substances which can cause breathing problems.

The medical has to be carried out before start of exposure and then at least yearly. This form does cover the information and medical examination necessary to assess potential problems to health. It can be used to be completed by the employee and medical parts can be assessed at most surgeries by their GP or practice nurse. Should any problems be highlighted the form can be sent to the address provided for assessment by an experienced Specialist in Occupational Medicine. A fitness certificate will be provided after the assessment.

The form can be used more than once within your company and does not include costs for the medical assessment and certificate.
