DSE Screening Medical form - Paper Version

The Health and Safety (Display Screen Equipment) Regulations 1992, requires employers to offer their employees working with DSE regular eye checks.

As certain medical conditions can influence vision, this medical form does trigger the relevant questions related with a comprehensive assessment. It is divided into 2 parts. Part 1 needs to be completed by the employee and part 2 by an optician. The form does provide an address for assessment by a qualified Occupational Health Practitioner. A fitness certificate will be issued after the assessment.

The form can be used more than once within your company and does not include costs for the medical assessment and certificate.
